Abstract

This study was conducted in co-operation between Tropical Medicine Department, Ain-Shams University, the Gastroenterology and Hepatology Department at El Agouza Police Hospital and The National Hepatology and Tropical Medicine Research Institute (NHTMRI). We performed this study on 50 adult Egyptian patients with proven chronic hepatitis C virus infection, group I; 25 chronic HCV infection with hepatocellular carcinoma and group II; 25 chronic HCV without hepatocellular carcinoma, that are fulfilling the eligibility criteria. Among the 25 chronic HCV without HCC enrolled patients (group I); there was male predominance being 18 males (72%) and 7 females (28%). Their age ranged between 35 and 70 years (mean 58.8 ±10.9 years). Concerning the clinical presentation, the majority of HCV cases (72%) were accidently discovered. Among the 25 chronic HCV with HCC enrolled patients (group II); there was male predominance being 19 males (76%) and 6 females (24%). Their age ranged between 45 and 69 years (mean 56.4 ±6.7 years). Concerning the clinical presentation, the majority of HCC cases (56%) presented with right hypochondrial pain. Concerning the diagnostic performance of AFP and AFM1 in differentiating HCC from HCV, AFP had significant excellent diagnostic performance while AFM1 had significant low diagnostic performance in differentiating HCV with HCC group from HCV without HCC group. Although aflatoxin is not a good diagnostic test but it is a good indicator of presence of HCC. It can be considered as an initial step in the diagnostic steps of HCC.
