Abstract The decision of ostomy or primary anastomosis of the small intestine and right colon in the presence of peritonitis is taken according to different factors as the severity and duration of the peritonitis, cause of the perforation, general condition of the patient, portion of the resected segment and presence of other risks. So ostomy is preferred in neglected peritonitis, bad general condition of the patient, and when the cause of the perforation is vascular or inflammatory (eg. Typhoid fever, Crohn’s disease and TB enteritis) or if the patient was on steroids or cytotoxic drugs or taking radiotherapy or chemotherapy. Otherwise primary anastomosis is preferred especially in proximal lesions due to less postoperative complications, no need to second operation and no difference in the risk of leakage. |
