Abstract

It is evident that while the immediate rate of pain relief of trigeminal neuralgia pain after percutaneous trigeminal radiofrequency gangliolysis (neuro-ablative procedure) is higher than that after trigeminal nerve micro-vascular decompression (neuro vascular decompression procedure ). But the pain recurrence rate is higher in the patients treated with PTRG than in patients treated with MVD, especially in the first five years post operative, while the complications rate shows low morbidity and mortality with variable encountered complications according to the used procedure favoring the percutaneous radiofrequency trigeminal gangliolysis as an initial less invasive procedure, but still there isn’t enough evidence to support the choice of one procedure over the other.
Difference in studies design, preoperative assessment of patients and definition of recurrence rate had a hindering effect on this systematic review. It was difficult to compare recurrence rate between series because of the different approaches to classify and define recurrence. In our analysis multiple definitions of pain recurrence were used across the involved studies.
In the absence of more definitive studies, there is insufficient evidence to recommend or reject any treatment modality for idiopathic trigeminal neuralgia. A well designed, multicentre prospective study with standardized preoperative clinical and radiological assessment, definition of pain relief, post procedure recurrence of trigeminal neuralgia, complications and definitive causes of procedure related mortality have to be warranted in order to provide confirmative data regarding establishing an efficient treatment protocol for idiopathic trigeminal neuralgia
