Abstract

In critically ill patients with systemic inflammatory response syndrome(SIRS),there is massive activation of the inflamatory cascade and excessive production of proinflamatory cytokines and lipid-derived inflamatory mediators.
Fish oil supplies omega-3 polyunsaturated fatty acids (PUFAs), particularly eicosapentaenoic acid (EPA), docosahexaenoic acid (DHA) and alpha linolenic acid (ALA) which have many anti infamatory effects as they compete with arachidonic acid (omega -6) for the conversion to lipid mediators, influence lipid-bound second messenger generation and dependent cellular functions, inhibit the production of interleukins(IL-1, IL-6 and IL-8) and tumour necrosis factor(TNF) ,decreased expression of some adhesion molecules on the surface of monocytes and macrophages and they provide a source for resolvins necessary for the resolution of inflammation.


Supplementation of omega-3 fatty acids lipid emulsion to critically ill patients including[surgical,septic and acute respiratory distress syndrome(ARDS) proved to be safe and provides clinical benefits as resolution of inflammation, improvement of gas exchange , reduced length of hospital stay and decrease mortality. Recent guidelines recommend regular use of omega-3 fatty acids to all critically ill patients especially ARDS patients.
