Abstract

Hepatocellular carcinoma (HCC) is a major health problem worldwide, with an estimated incidence ranging between 500,000 and 1,000,000 new cases annually. It is the fifth most common cancer in the world, and the third most common cause of cancer related death. (Lau et al., 2008) Each treatment modality for HCC can be considered to have one of three goals: cure, local control and bridge to transplantation, and palliation. For years, partial hepatectomy and liver transplantation have been considered as the main curative treatments. Various loco regional therapies are used for patients who are not candidates for surgical cure because of severity of liver disease or advanced stage of HCC (Lai et al.,2011)
This study included 30 patients presented to general surgery department in Minya university hospital diagnosed as single lesion HCC. Patients divided in two group, group (A) and include 14 patients for whom surgical resection was done ( Hepatectomy),another group (B) included 16 patients and for whom Percutaenous Radiofrequency ablation was done between January 2012 and January 2014.
The aim of the study to assess and evaluate the treatment outcome of open hepatic resection versus percutaneous Radiofrequency Ablation regarding their feasibility, radicality and availability of both techniques.
For every patient, the following data were recorded:
Pre operative lapaoratory investigation i.e. (liver function, CBC, renal function and alpha feto protein. Tumor locations, tumor size (which detected by Ultrasonography and Triphasic CT Scan) child-Pugh class, associated diseases. Post- procedure outcome, complications and incidence of recurrence.
Our result show that difference in two group as regard procedure time , blood transfusion requirements and hospital length stay in favor of percutaneous Radiofrequency Ablation . Complication rate in RF group (n=16) occur in six patients (37.5%). in the form of subcapsular hematoma , intraperitoneal hemorrhage , biloma , pleural effusion and arterio – venous shunt. But in resection group (n=14) complication occur in 4 patients (28.6%). In the form of bleeding, bile leak, Surgical wound infection and incisional hernia. As regard tumor recurrence rate in two groups are two cases (12.5 %) in RF group (n=16) and zero case in resection group.
So, surgical resection improves the overall survival and recurrence-free survival in comparison with radiofrequency ablation. New evidences are needed to define the real role of the percutaneous technique as an alternative to surgery.
