Abstract

This case control study conducted to assess feeding tolerance in 41 late preterm infants (LPIs) recruited from NICU department, Ain Shams University Hospital, and divided into two groups:
• The first group included 21 late preterm infants (LPIs) received expressed breast milk (EBM).
• The second group included 20 late preterm infants received premature formula.
Regarding gender distribution, there were 23 female infants (56%) and 18 male infants (43%). All infants were subjected to full history taking, clinical examination including anthropometric measurements (body weight at birth and study time, body length and head circumference). The investigations done included laboratory investigations (HGB%, WBCs, PLTs and CRP) and radiological by serial U/S measurement of antral cross sectional area before and after milk feeding.
The statistical analysis of the results of this study revealed a statistically significant difference between the two groups regarding mode of delivery by either normal (NVD) or cesarean section (C.S).
Regarding anthropometric measurements, there was a statistically significant difference between the two groups in body weight at birth (BWs) and head circumference (HC).
A statistically significant difference present between the two groups regarding U/S values (ACSA) in pre-feeding value, 2nd interval (half emptying) and 3rd interval.
Regarding the first group (control group) with expressed breast milk, there was significant positive correlation between full gastric emptying (4th interval) and post natal age (days).
Also a significant positive correlation in the same group between 3rd interval and both of gestational age (weeks) and post natal age (days).
There was highly statistically significant difference between ultrasound values in the first group in:
• Pre-feeding value with 1st interval.
• 1st interval with 2nd interval (HET).
• 2nd interval (HET) with 3rd interval.
In the other hand, regarding the second group with premature formula there was highly statistically significant difference between ultrasound values in:
• Pre-feeding value with 1st interval.
• 1st interval with 2nd interval (HET).
• 2nd interval (HET) with 3rd interval.
Finally, the current study showed faster gastric emptying in control group with expressed breast milk than the second group with premature formula. The nature and composition of milk play an important role in the physiology of gastrointestinal motility and gastric emptying of the stomach.
