Abstract

Inguinal hernia repair is the most frequently performed operation in general surgery, Nearly 800 000 patients undergo surgery for inguinal hernia repair in the United States each year, making this disease a major public health issue there has been a great upsurge in interest in hernias over the last 15-20 years sparked by the introduction and wide-spread use of prosthetic mesh closely followed by the advent of laparoscopic surgery.
Because recurrence is rare among patients who receive wall reinforcement with prosthetic mesh (0%–2%) and because the sensation of a foreign body is not an important limiting factor, chronic pain is now the most important complication associated with hernia mesh repair. In fact, post hernioplasty pain can significantly influence the patients‟ quality of life. Moreover, some authors report that a significant proportion of the pain is of neuropathic origin. Because of this, when genito-femoral, ilio-inguinal and/or ilio-hypogastric nerves are damaged by suture entrapment or by contact with mesh, the probability of acute or chronic pain after surgery is very high. The potential complications associated with sutured and stapled mesh fixation have led surgeons to consider an alternative method of mesh fixation.
Because of its haemostatic and adhesive properties, fibrin glue has been used in many areas of surgical treatment in recent years. One example is hernia repair, where fibrin glue has become increasingly established as an alternative method for mesh fixation. Many experimental and clinical studies have reported the use of glue for prosthetic mesh fixation during open inguinal hernia repair. Clinically, fixation with fibrin glue shows a reduced postoperative complication rate compared to other fixation methods (staples, sutures), particularly as regards to pain.
