Abstract

Psoriasis is disease of the skin that is characterized by a chronic relapsing nature and variable clinical features, it affects 2-3% of the northern hemisphere. Treatment of psoriasis is based on symptomatic therapy that is able to induce remission of the disease, no therapy is yet able to induce permanent remission.The mechanism of action of methotrexate is based on its behavior as a competitive inhibitor of dihydrofolate reductase, which is essential for the reduction of dihydrofolate to tetrahydrofolate. Thus, by blocking thymidylic acid synthesis methotrexate blocks DNA but not RNA synthesis and thus in hibite cellular replication. The drug is cell-cycle specific and active in the ”S” phase of the cycle. In this work, methotrexate was not used in the free state but as gel- liposomal encapsulated methotrexate (Gel-LMTX). The gel liposomal methotrexate, LMTX. was subjected to laser irradiations of enegies 30,60, and 80 Joules to increase the rate of release of the drug. The stability of the deug was investigated by measuring Fourier transform Infrared (FTIR) spectra and that of liposomes by thermal analysis (DSC). Liposomal methotrexate has been farrnulated in a gel formulation, a technique developed in this work, to facilitate the drug penetration into the skin. In this work a new strategy has been developed for the use of MTX as a treatment for psoriasis. The LMTX in gel formulation is used as a topical treatment. This new strategy has led to the exclusion of almost all the side effects of the use of MTX . To strat with, gel-methotrexate was applied (0 albino mice. Clinical and pathological investigation indicated clearly that drug did penetrate into the skin. In this work gel-LMTX was applied to 40 patients, 16-females and 24- meles with psoriatic lesions in different sites: Abdomen, scalp, palms., and forearms. In most cases , the complete clearance of the lesion starts after 2-3 months of treatment, complete clearance was observed after a period of 4-10 months of treatment, complete clearance was observed after a period of 6-8 months of follow up . The results of this work show a percent recovery of the order: 40% for palm psoriasis, 50% for both scalp and lower limp, 70% for upper limp and 90% for both abdomen and elbows psoriasis.Of the main conclusions of this work are: Liver functions are not affected by using MTX as a drug with the adopted strategy. Kidney function and complete blood picture are not affected by using MTX as a drug with the adopted strategy Complete clearance is obtained for localized and recent lesions.
