Abstract

βTM is a common chronic hemolytic anemia in Egypt , that have a wide variety of clinical phenotypes, ranging in severity from clinically silent heterozygous β-thalassemia to severe transfusion-dependant thalassemia major.
Abnormal lipid profile patterns have been suggested to occur in thalassemic patients and also in various hematological disorders including sickle cell disease, glucose-6-phosphate dehydrogenase deficiency, spherocytosis, aplastic anemia, and myelodysplastic syndrome.
Childrens with β-thalassemia are at risk of developing premature atherosclerosis.
The aim of this study was to assess whether there is any change in lipid profile in patients of β-thalassemia major that may predispose to early atherosclerotic changes and hence, whether there could be any medical intervention to delay or slow this process.
The current study was conducted in the pediatric Hematology clinic in Beni Suef University Hospital , it included 40 thalassemic patients,their mean age was(9.4±4.4 years),19females and 21males.They subdivided into two groups:group(a) included 21 children that splenectomized ,group(b) included 19 children that not splenectomized.
TwentyFive children were matched in age and sex with the studied group of patients ,they were recruited in this study as control group.

All patients were subjected to the following:full history taking and laboratory investigations including complete blood picture,serum ferritin,serum total cholesterol,serum low density cholesterol,serum high density cholesterol,serum triglycerides,serum apolipoprotien A1,serum apolipoprotienB and then APOB/APOA1calculated.
As regard lipid profile we fou significantly lower serum(Cholesterol,HDL,LDL),Higherserum(TAG,APOBandAPOB/APOA1ratio)(P=0.001,0.006,0.001,0.016and0.001respectively) and significant difference in APOA (p=0.547).
In patient group,a positive correlation was observed between serum triglycerides and frequency of blood transfusion(r=0.4,p=0.006),serum ferritin(r=0.4 ,p=0.020) and also positive correlation between LDL-C and serum total cholesterol(r=0.8 ,p=0.001) was found.
Finally ,we concluded that βTM childrens have lipid abnormality values in form of,low serum TC,LDL-C,HDL-C values,high serum TG ,APOB and increased APOB/APOA1 ratio ,but didn’t exceed normal values.
