Abstract

This work aimed to:
1. Assess the prevalence of sexual dysfunctions among married female diabetic patients in Beni-Suef.
2. Assess the effect of diabetes mellitus on different aspects of female sexual function (FSFI domains).
Using the Arabic translated validated version of female sexual function index (FSFI).
Methods of the study
The present study included 155 married women who were interviewed to answer FSFI questionnaire. The women comprised two groups. The first group included 100 diabetic patients (diabetic group), while the second (control group) included 55 non-diabetic women. Diabetic women were recruited from those attending the Diabetes and Endocrinology outpatient clinic at the university hospital of Beni-Suef. The control group were normal women (not known to be diabetics) recruited from the outpatient clinic of dermatology department, Beni- Suef University Hospital who were accompanying their sick children.
Parameters of patients’ evaluation included: Personal history, history of medical diseases, complete history of diabetes (duration, medication) and FSFI questionnaire for both groups. Women in both groups were subjected to laboratory investigations (Fasting blood sugar, 2-hour postprandial and glycosylated hemoglobin) and arterial blood pressure measuring.
The study revealed the following results:
It was found that the mean age of the patients was 40.5±7.8 years with a range of (24-50) years in diabetic group and was 37.2±8.4 years with a range of (19-50) years in the control group. The diabetic patients were older than the non-diabetic women. There was a statistically significant decrease in the mean of desire, arousal, lubrication, orgasm and satisfaction domains and total score of FSFI in diabetic patients compared to control candidates. The prevalence of Female Sexual Dysfunction (FSD) was statistically high significant greater in the diabetic group than the control one.
In general, individual domain scores were less in the diabetic group than in the control group of the same age-range; but without any statistical significance in Patients less than 30 years and high significant in age sub-group who having 30 to 39 regarding domains scores and FSD while in those having 40 to 50 years significant relation found only in FSD percent.
Comparison between the three age sub-groups in the diabetic and control group showed statistically significant decrease in all domains and total score with increasing age and FSD increases significantly with increasing age. Obesity had a significant effect on mean of total FSFI score but no significant effect on FSD in the diabetic patient. Educational level had no effect on FSD.
Regarding diabetic duration there were non-significant decrease of FSFI scores and non-significant increase in FSD with the increase in the duration of diabetes. There were no significant differences between type I and type II of diabetes with respect to FSFI scoring and FSD percentage.
FSD was non- significantly greater in hypertensive than in non-hypertensive diabetic women but FSFI scoring was significantly lower in hypertensive. Peripheral neuropathy was associated with significant lower FSFI scoring, and a significant increase in FSD percentage.
It is concluded that FSD was higher with a statistical significance in diabetic more than non-diabetic women. Desire, arousal, lubrication, orgasm and satisfaction domains were significantly lower in diabetic patients compared to control subjects. So the female sexual dysfunction is a common complication in diabetic women. Age (with increasing age of the women) and peripheral neuropathy were associated with a significant increase in FSD. On the other hand, obesity, hypertension, type and duration of diabetes and educational level had no significant effect on FSD.
