Abstract

Pulmonary tuberculosis is a major cause of morbidity and mortality resulting in the great number of deaths due to only one single infectious agent.
Drug resistance threatens global tuberculosis control efforts.
The present study included 25 patients admitted to Abbassia Chest hospital. The patients were resistant to at least rifampicin and INH.
It was done during the period from January 2010 to December 2010. Their files were analized and the following data were discussed from the files.
1.Full medical history:
-Source of infection.
- Age: In this study there were 18 males and 7 females. The mean age was 40.36 year.
-Occupation: According to the occupation of the studied group,employed patients were 68%while unemployed were 32%.
-Co-morbidities associated with MDR-TB: were C.O.P.D, it was 20% and diabetes mellitus was 8%.
-Wether primary or secondary resistance: According to type of resistance acquired resistance was 96% primary resistance was4%.
-where did the patients receive anti-TB treatment: in Hospital or at home.
-Whether patients smear positive or negative.
-Pulmonary or extra pulmonary T.B: In the studied group pulmonary TB cases were 100% and there is no cases of extra pulmonary TB.
-Name and duration of anti-TB drugs.
-Patients compliance.
-Follow up of anti-TB treatment.
2.Types of the patients:
New, Relapse, defaulter and treatment failure.
- According to type of patients new cases were 4% and re-treatment patients were 96%.
- In the studied group, fate of previous anti-TB treatment were treatment failure 76%, Defaulters were 20%, new cases were 4%.
3.Clinical examination.
4.Radiological examination: chest X-ray.
5.Laboratory investigation:
 CBC – ESR – Creatinine – SGPT – FBS – Direct Zeihl Neelsen smear of sputum – Culture and sensitivity of Lowenstein – Jensen media of sputum.
6.Drug sensitivity test to Anti-TB drugs: In the studied group:Results of conventional drug&sensitivity test shows that resistance to isoniazid and rifampicin were 100%, resistance to streptomycin was 88%, resistance to ethambutol was 76%.
7.Rapid rifampicin test:The results of Rapid Rifampicin test shows that only 22 patients in the study group(25 patients)are MDRpatients.
8.Regimens of resistance used.
9.Complications of drugs: In the studied group complications of drug were Gastritis 52% depression 12%, Hyperuriceamia 16%, 12% hyperlipideamia electrolytes disturbance 12%, others as arthralgia was 16%, HTN 4%, Alopecia areata 4%.
