Abstract

Intramedullary nailing isn’t a new idea it belongs to decades of trials and imporvemnts that made the idea withstand and in some times overcome other fixation methods .
Proximal femoral fractures mainly occurs in the elderly with low momentum falls specially in intertrochanteric on the contrary of subtrochanteric fractures which occurs mainly with the high energy trauma in young patients.
Wide varieties of side plates have been introduced for proximal femoral fractures with a wide spectrum of modifications in competition with the intramedullary nailing
Intramedullary nailing in proximal femoral fractures has a wide variety of types that made the surgeon‘s decision easier that it correlates with every type of proximal femoral fractures.
The generations of the intramedullary nails and their modifications are widely spreading and have different lengths and locking system.
Intramedullary nails has proved a victorious privilege over sideplates in subtorchanteric fractures and unstable intertrochanteric fractures.
Stable intertrochanteric fractures were thought for a long time to be treated exclusively with side plates of sliding hip screws but newer generations of both side plates(locked compression plate) and intramedullary nails 3rd and 4th generations proved competency with each other way over the old dynamic hip screw.
Intramedullary nailing concept seems to prevail in proximal femoral fractures except for minimal competencies from the new side plates like the PCCP and LCP.
Intramedullary nails with its latest mofications like the PFNA with augmentation options seems to have the future of proximal femoral fractures fixation throne.
