Abstract

The diagnosis of PAH is challenging but can be supported by the presence of lower PDW and MPV. Hematology indexes are simple markers not requiring advanced or expensive technology.
In conclusion, our study revealed significantly decreased MPV and PDW in children with CHD but the decrease was more significant in CHD with PAH than CHD without PAH. In addition, we observed negative correlation between PDW and MPV in children with PAH, but PDW showed significant correlation with many clinical parameters as hight, weight, BMI, heart rate, systemic systolic and diastolic blood pressure. so PDW is more valuable than MPV.
Platelet’s indices must be followed up carefully in patients with CHD for early detection of PAH in rural areas where as echo is not available.
Studies are needed to address the role of platelets in patients with PH, especially those with PH caused by different etiologies, and the possible role of treatments specifically targeting platelet activation.
Another more advanced study involving PDGF pathway in patients with PAH to elucidate its role in pulmonary vascular remodling process responsible for PAH progression.
Further study to detect role of platelete in prognosis of congential heart disease left to right shunt.
