Abstract

Hypocalcaemia is an important and common complication following thyroid surgery. In most cases, post-thyroidectomy hypocalcaemia is temporary, but may take several months to resolve.
Despite being self-limiting in most patients, symptomatic hypocalcaemia is of particular concern because of a delay in its manifestation and the consequent need for prolonged patient hospitalization or readmission.
Development of post thyroidectomy hypocalcaemia is multifactorial and sometimes difficult to prevent.
One of the common causes of hypocalcaemia is inadvertent removal of parathyroid glands or vascular comprise to the parathyroid glands after surgery for thyroid disease.
Surgical inexperience also associated with an increase in the incidence of hypocalcaemia post-thyroid surgery.
Hypocalcaemia can present as an asymptomatic laboratory finding or as a severe, life-threatening condition. In the setting of acute hypocalcaemia, rapid treatment may be necessary.
The hallmark of acute hypocalcaemia is neuromuscular irritability. Patients often complain of numbness and tingling in their fingertips, toes, and the perioral region.
Muscle cramps can be very painful and progress to carpal spasm or tetany. In extreme cases of hypocalcaemia, bronchial or laryngeal spasm may occur.
Our study used routine oral calcium and vitamin D supplements to prevent the development of symptomatic hypocalcaemia and to increase the likelihood of early hospital discharge after bilateral surgical treatment of the thyroid gland or exploration of the parathyroid glands.
The study showed that routine oral calcium and vitamin D supplements reduce the incidence and severity of hypocalcaemia after total thyroidectomy.
