Abstract

M
iscarriage is the termination of pregnancy by any means before the fetus is sufficiently developed to survive.
It is an extremely common place occurrence because one out of four or five pregnant women has vaginal spotting or heavier bleeding during early gestation, of those approximately one half will abort.
Unfortunately spontaneous expulsion of non-viable pregnancies is frequently delayed for weeks following the onset of clinical symptoms,leading to problems of prolonged vaginal bleeding,infection and complications of mother anxiety.
As the bleeding in the first trimester is one of the most frequent complications in pregnancy. The presence of reliable test for the prediction of outcome of pregnancy in such condition is very important to determine which patient should be prepared for the poor prognosis of their current pregnancy and spare them the expenses and complications of anxiety that may ensue prior to definitive therapy and on the other hand to determine which patient should be reassured regarding the favorable prognosis of their pregnancy outcome and encouraged to follow the instruction of their obstetrician and to rest mentally and physically.
Ultrasound HCG and progesterone titres are widely used to assess the risk of miscarriage at the early stages of pregnancy. Though very useful, they are not considered as satisfactorily predictors of imminent abortion during the first three months of pregnancy.
The aim of this study is to estimate the prognostic efficacy of maternal serum CA125 in first trimesteric threatened miscarriage.
The study was conducted at Ain Shams University Maternity Hospital in period from the first of January 2013 to the first of January 2014.
The study included 180 pregnant women in their first trimester, they were divided into two groups:
Group I: included 120 pregnant women with symptoms of threatened miscarriage (vaginal bleeding).
Group II: included 60 non complaining pregnant women in their first trimester.
They all were chosen according to eligibility criteria and were subjected to careful history taking, clinical examinations and ultrasound examination for ensuring positive fetal life and closure of the cervix. The results of the study showed that:
There were no significant statistical difference among all groups with regard to maternal age,parity and gestational age (p more than 0.05) but there was statistically significant difference among all groups with regard to number of abortions(P < 0.05).
The serum CA125 levels showed highly significant results regarding the prognosis of first trimester threatened miscarriage. There was statistically significant difference between the study group (group I) and the control group (group II).Also there was a statistically significant difference between continued and aborted subgroups regarding serum CA125 levels (P <0.05).this study suggests the possibility of taking 58 IU/ml as a cut off value for serum CA125 to differentiate between threatened continued and aborted subgroups. This cut off value shows sensitivity of 78 %and specificity 97%.
