Abstract

This study was conducted on fifty women with primary postpartum hemorrhage (PPH) unresponsive to medical treatment. The study participants were recruited from delivery room in the Obstetrics &Gynecology department at El Menoufia university hospital in the period between may 2011 to June 2012.
During our study our device was inserted vaginally in 47 cases (94.0%) and abdominally in 3 cases (6.0%). On an average 150-300 ml with mean of 200 ml of normal saline was required to create adequate tamponade to stop the bleeding.
Our device has multiple advantages as
1- Inexpensive and available in low resource area.
2- can be inflated with air or fluid and when inflated can reach large size to fit well in the gravid uterus (may exceed 700 ml).
3- Sterile, disposable, easily prepared by general practionar and general obstetrician,
4- short tip of drainage channel to avoid perforation .
5- wide bore of drainage channel to avoid obstruction by blood.
The unique advantage of our device is that it is the first condom catheter in the world that have drainage channel simulating bakri catheter with very low cost and similar effect.This drainage channels make suturing of the cervix to prevent displacement not necessary and can be done with vaginal pack.
from this study despite the small number of included patients we may conclude that. Condom catheter effectively controls the PPH. It is a simple inexpensive and safe method of conserving the reproductive capacity along with saving the life of women with intractable PPH and condom catheter (Saif modification) can be important especially in many obstetric settings in the developing world where uterotonics, safe blood, anesthesia and experienced obstetricians / surgeons are not readily available all around the clock. from this study despite the small number of included patients we can recommend adoption of this technique and dissemination of knowledge about this option may save lives in many settings worldwide as the procedure is very simple and safe.
