Abstract

Knee osteoarthritis (KOA) is one of the commonest rheumatic disorders in Egypt especially among females. The severity of radiographic KOA is usually assessed through Kellgren-Lawrence (K-L) scale. In advanced stages, the patients suffer from severe pain and restriction of mobility which contribute to physical disability and reduction of Health-Related Quality of Life (HRQoL) that is important for patients and health care professionals including nurses who are in direct contact with KOA patients. Nurses need to understand the nature of relation between the structural changes in KOA on the one hand and changes in both functional performance and HRQoL on the other hand. This will enable nurses to develop self-care programs with targeted strategies for slowing the disease progression, preventing the functional decline and for improving HRQoL. Thus, the aim of this study was determining the relationship between the radiological gradings and both func¬tional performance and HRQoL among women with KOA.
Setting:
The study was conducted at the Physical Medicine, Rheumatology and Rehabilita-tion Department in Alexandria Main University Hospital on sixty women with confirmed diagnosis of unilateral or bilateral KOA.
Tools:
Three tools were used for data collection:
1- Radiological grading check list for Women knee osteoarthritis including biosociodemographic data and Kellgren and Lawrence (K & L) radiological grading system.
2- Functional Performance Checklist for Women with Knee Osteoarthritis for recording the time taken for- the fast 50 meters walking, going up and down seven steps without stopping, and completing 3 sets of standing-sitting from an armed chair without stopping- in seconds and an assessment of perceived difficulty following the performance of each test.
3- Health- Related Quality of Life questionnaire for Women with Knee Osteoarthritis that measured the effect of KOA on the following : pain severity, stiffness, physical function, social function and emotional function. Questionnaire’s scoring system was used then total score was calculated by summing up the total scores of the five subscales; in case of women with bilateral knee involvement, the score taken for pain was that for the most painful knee; and that score for stiffness was taken from the knee with marked stiffness. The total scores ranged from 0 to 124. The lower the score, the better the is the HRQoL.
