Abstract Objectives: To estimate alpha 1-acid glycoprotein as indicator for the diagnosis of early onset neonatal sepsis in full term neonates and its relationship to the outcome of the patients. Design: prospective case control study. Setting: This study was done in the department of neonatal intensive care units.university hospital and general hospital, Beni-suef,Egypt. Subjects: 45 neonates divided into 30 patient cases have positive blood culture and 15 healthy neonates for comparison. Methods: History, clinical examination and laboratory were performed to all cases. Results: The most frequent symptoms and signs considered in sepsis was temperature instability, followed by poor feeding followed by signs of respiratory distress. The most frequently cultured organism was E coli (7 cases) followed by both klebsella and staph aureus each one (6 cases). There was a significant difference between patient and control groups regarding the mean value of α1-AG (P<0.05). Conclusion: from this study we can conclude that alpha 1 acid glycoprotein is a good diagnostic marker for detection of neonatal sepsis.
