Abstract

The management of epithelial ovarian cancer differs between centers in different countries that could be attributed to many factors including differences in financial resources, treatment guidelines, availability specialist surgeons performing surgery and lack of good clinical trials.
In this study we evaluated our management of epithelial ovarian cancer and we used data from Nottingham oncology center from the UK as a comparator (after permission from Dr. Stephen Chan). The study included 83 patients from oncology department, Menoufia University and 151 patients from Nottingham oncology center, University hospital, UK.
The Patient’s clinicopathological characteristics, treatment modalities, response and survival outcome in both centers were compared and the Pearson chi. Square test and fisher’s exact test used to determine the significance.
Regarding clinicopathological characteristic, there were significant differences in median ages and age ranges (higher in Nottingham center). There was nearly similar percent of pathological subtypes except the absence of clear cell pathology from Menoufia center like the oriental data reported by (MECC), high grade tumors were the predominance in both centers, and common presentation of Menoufia oncology center patients was late stages (84.3% were stages III and IV) which not only was significantly higher than Nottingham center but also the highest among most of studies mentioned issue of stage.
All patients in Nottingham oncology center underwent debulking surgery and also all surgeries done by specialist gynecologic oncologist while 13.3% of Menoufia patients didn’t do debulking surgeries and no surgery done by specialist gynecologic oncologist and a small higher in the percent of the patients underwent optimal debulking surgery in Nottingham oncology center than in Menoufia center (58.9% and 53% respectively) and the difference was statistically insignificant (P = 0.381).
Multidisplanary team including specialist gynecologic oncologist in Nottingham center allowed for proper selection of the patients underwent debulking surgeries and we thought that this was the rational of the significant difference in the percent of the patients underwent primary surgery in Nottingham center than Menoufia center (89.4% and 59% respectively) (P = 0.000).
Significant higher percent of the patients received single agent carboplatin in Nottingham center than in Menoufia center (41.7% and 13.3% respectively) (P = 0.000).
Patients shared in clinical trials were only in Nottingham center (7.3%) and lack of financial resources is the main reason for the absence of clinical trials in Menoufia.
The percent of platinum sensitive patients in Menoufia oncology center (74.7%) was higher than that in Nottingham center (67.5%) and this difference was statistically significant (P = 0.039).
