The third tool was observation checklist for monitoring daily living activities. The last tool is an observational checklist for monitoring home environment. Tools were used pre, post program and after 3 months. Results mean score of knowledge for stroke patients preprogram was (1.38) which improved after the program to (8.04) then decreased to (6.9) after 3 months. The total mean score of the practice for stroke patients preprogram was (7.42) then improved to (12.7) after the program and further improvement to (14.7) after 3 months. There was a statistically significant relation in activities in daily living in pre, post and follow up the program. Conclusion lack appropriate patients and caregivers knowledge and practice preprogram and the majority of them were dependent in motor activities Recommendations continuous training program for stroke patients and their caregivers should be implemented to decrease the complications and prevent recurrent attacks.
