Abstract

Currently, WHO classification 2008 designates (Grey zone lymphoma) as mediastinal involvement with lymphoma intermediate between diffuse large B-cell lymphoma (DLBCL) and classical Hodgkin lymphoma (cHL). It is claimed to have more aggressive course than both cHL and DLBCL. Similar extramediastinal intermediate cases do exist and currently are on record. Urged by scarcity of data, we proposed this study to explore clinicopathological features of these cases and compare them with those of both cHL and DLBCL. Methods and Results: Thirteen cases of extramediastinal lymphoma cases with Grey zone lymphoma features were identified over a 9-year period. The control group included 183 cases of cHL and 77 cases of DLBCL. Patients were characterized by elderly-onset, poorer performance status, lower complete remission rate, higher 2-year mortality rate and poorer overall prognosis. HRS-like cells of were CD30+ with a higher rate of CD20+ (100%) and lower rates of CD15+ (25%) and Fascin+ (50%) than cHL. Conclusion: Extramediastinal grey zone lymphoma is a distinct entity of aggressive lymphoma characterized by intermediate histological and immunophenotypical features between cHL and DLBCL but with male predominance, older onset and worse prognosis than both of them. Those cases showed better response to anit-CD20 (Rituximab). We propose inclusion of the term “extra-mediastinal grey zone lymphoma of the elderly” (EMGZLE) to the new WHO classification as a more comprehensive term that echoes the underlying clinical and biological features of such cases.
