Abstract The present study was carried out in nephrology department of Ahmed Maher Teaching Hospital in the period between March and May 2012.The study included eighty end-stage kidney disease patients on regular heamodialysis presented by hyperphosphatemia (phosphorus level more than 5.5 mg/dl ).They were divided into 2 groups each containing 40 patients with high-flux polysulfone membrane surface area 1.6 m2 as classified :- The patients be divided in to two main groups. • Group 1: 40 patients will receive a dose of dialysis for 4 hours. • Group 2: 40 patients will receive a dose of dialysis for 6 hours. The duration of the study is 3months.Serum calcium, phosphorus , albumin, parathyroid hormone and alkaline phosphatase enzyme are measured before and after the study to compare the effect of this methods on reducing serum phosphorus level, medical consent were taken from all patients before the starting of the study. Inclusion criteria:- -Hyperphosphatemic , phosphorus level more than 5.5 mg/dl . All subjects submitted to:- (1) Detailed history taking. (2) Clinical examination. (3) Laboratory investigation:- [Serum calcium, phosphorus ,Albumin, Parathyroid hormone, Alkaline phosphates enzyme]. Before and after the performance of the study (4) The parameters of the heamodialysis session are:- - Pump: 300 ml/min. - Dialysate flow rate: 500 ml/min. - high phosphorous clearance filter (high-flux polysulfone with membrane surface area 1.6 m). -Time of session: 4 hours in group I time is prolonged to 6 hours in group II . (5) Usage the same dose of calcium carbonate and calcium acetate as a phosphate binder (1.5 g / day). (6) The collected data were tabulated and analyzed using the suitable statistical methods. Results of the study showed that with increase time of dialysis from 4 to 6 hours serum phosphorus and PTH showed marked improved ( of high significant value ) with improvement of clinical symptoms like itching , also corrected calcium and calcium phosphorus product increased bur on non signuficany value. |
