Abstract

Rheumatic heart disease is a main health problem in Egypt and mitral stenosis is a main component of it. In our study we aim to uncover the incidence of LV systolic dysfunction in patients with predominant rheumatic mitral stenosis. One hundred and four patients were included in our study. We excluded any patient with any obvious cause affecting LV systolic function (MR more than mild, other valve affection, cardiomyopathies, hypertension, diabetes and coronary artery disease). All the patients were assessed by 2D echocardiography and assessed by M-mode and Simpson’s method to detect EF. Patients with LV systolic dysfunction were assessed by coronary angiography.
The results showed that there is a low incidence of LV systolic dysfunction in patients with isolated rheumatic mitral stenosis but this incidence doesn’t correlate with the severity of mitral valve affection and falls greatly with good exclusion of patients with other cardiovascular disease affecting the LV systolic function.
The results raised a question about the role of hidden coronary artery disease in LV systolic dysfunction affecting patients with isolated rheumatic mitral stenosis
