Abstract

Shared governance has attracted the attention of nursing over the last decade in response to maintaining nursing job satisfaction and quality of care. Shared governance has been identified by Register Nurses as a key indicator for excellence in nursing practice and allows nurses significant control over major decisions about nursing practice. Work satisfaction used to describe whether employees are happy and fulfilling their desires and needs at work employee satisfaction is a major factor in employee motivation, goal achievement and positive employee morale in the workplace.
The present study aims at assessing head nurses perception regarding shared governance, and Identify effects of shared governance on head nurses work satisfaction at Assuit University Hospitals. The study was includes all head nurses (No=168) working at all units of Assiut University Hospitals (Main, Pediatric and Women Health Hospital).
Tools of the study:-
A – Personal & Job characteristics sheet:
Including information about head nurses such as gender, age, educational level, unit & years of experience in the hospital.
B - Shared Governance Questionnaire:
It was developed by Ramo (1998) & Red book (2001), consisted of 26 items classified into seven categories: Climate for governance, organization communication, the board’s role, the president’s role, the organization’s role, joint decision-Making, assessing structural arrangements for governance. The responding of all items was (1) for Yes and (0) for No.
C- Work Satisfaction Scale Nurse (WSSN):-
It was developed by Stumpf scale (1987); it was consisted of 32 items, the responding scoring system was measured by 3 points likert scale. Ranging from (1) for dissatisfied to (3) for Satisfied.
An official permission was obtained to collect data from directors of the Assiut University Hospitals (Main, Pediatric and Women Health Hospital), official permission was obtained to collect necessary data from general director of Nursing Administration Department at Assiut University Hospitals and Oral consent taken from the participatory Head nurses in the present study. After reviewing the available literatures concerning the topic of the study, Translation of the tools from English to Arabic was done. It took about Four months from June to September 2010.A pilot study was carried out to assess tools clarity and applicability. Moreover, to identify problems that may be encountered during the actual of data collection. It applied on twelve head nurses from different units of the hospitals. Data collected from the pilot study analyzed and necessary modifications were done prior to the final application of the study tool. The data was collected by the researcher through structured interviewing with head nurses and the questionnaire sheet is supervised self administered by each participant. The researcher had explained the questionnaire to each one. The time took with each participant about twenty minutes. The whole data collection period took about three months from October to the end of December (2010).
The Result of the Present Study Revealed that:
- More than two thirds of head nurses were females and aged less than 35 years old, more than two thirds of head nurses had bachelor degree, the majority of the study subjects were married, and more than two thirds of head nurses had years of experience from 5 to less than 10 years.
- A significant statistical difference as regarding items of shared governance, climate for governance, organizational communication, organization’s role and board’s role.
- The highest mean score of items of shared governance in pediatric hospital in the study subject president’s role, joint decision making, board’s role, organization’s role, and assessing structural arrangements for governance and climate for governance.
- The highest mean score of items organizational communication in women health hospital, followed by in pediatric hospital and finally in main hospital.
- A statistically significant differences in the item of climate for governance in all hospitals in the study.
- A positive correlation between shared governance item (Organization’s role and Joint decision making) and highly positive correlation Joint decision making, followed by Board’s role and finally Organization’s role.
- A positive correlation between shared governance and work satisfaction items; there was highly statistically significant difference.
In the light of the results of this study, the following recommendations are suggested.
1. The board of director should give the head nurses opportunities to their area responsibility participate in goals setting and degree of autonomy with regard to increase their commitment toward goal achievement.
2. Providing in service training workshop for different nurse managers about shared governance.
3. Choosing the managers from the beginning should be by the majority vote and not depend on their profession.
4. Building an incentives system for managers with a fair manner to participate in achieve high level of satisfactions.
5. Building a climate of shared governance among nurse managers through providing of communications among organization constituents open and carried out in good an atmosphere of trust.
6. Encouraging the organizational communication through Consultation by the administration personnel that allows time and mechanisms for leadership to their constituents before offering recommendations, and have timely access to information necessary for organization members to give input into governance processes.
