Abstract

This study was conducted in the Department of Hepatobiliarypancreatic Surgery, Kyoto university hospital on 195 patients with hepatocellular carcinoma who underwent histologically curative resection during the period from January 2004 to January 2012. Patients were 144 males (73.8%) and 51females (26.2%), with a mean age of 63 ± 10.8 years (range from 29 to 83 years). Right hypochondrial dull aching pain was the most common symptom and occurred in 70 patients (35.9%), loss of weight in 51 patients (26.2%),abdominal mass in18 patients(9.2%), and jaundice in 9 patients (4.6%). 177 patients (90.8%) were Child’s Pugh class A and 18 patients (9.2%) were Child’s Pugh class B. HCV antibody was positive in 143 patients (73.3%); while HBsAg was positive in 39 patients (20%). Only 13 patients (7%) were negative for HBsAg and HCV markers All patients underwent surgical resection after complete laboratory and radiological evaluation. Major hepatic resection whether right liver lobe resection or left liver lobe resection was done for 116 patients (59.5%). Minor hepatic resection was done for 79 patients (40.5%). 163 patients (83.6 %) had conventional surgical approach and 32 patients (16.4%) had anterior approach. The mean blood loss was 1318.6 ±687.8 ml with 111 patients (56.9%) had a blood loss more than 1 liter. 96 patients (49.2%) received blood transfusion. Vascular reconstruction was performed in 7 patients (3.6%). The mean operative time was 3.1 ± 0.9 hours.
