Abstract

B
reast cancer is the most common malignancy in women and the principle cause of death from cancer among women globally, world wide breast cancer comprises 22.9% of all non melanoma skin cancers in women. Breast cancer can be classified by different schemata classification aspects include stage (TNM), pathology, grade, receptor status and the presence or absence of genes as determined by DNA testing.
Breast cancer diagnosed by the triple assessment method which is combination of clinical, radiological and pathological evaluation of a breast lesion.
Breast cancer clinically presented mostly by a lump, it may also presented by changes in breast size, shape skin dimpling, nipple inversion, discharge. Occasionally breast cancer presents as metastatic disease can mainly in the bone, liver, lung.
Breast cancer includes mammography, breast U/S and MRI.
Early detection of breast cancer may improve the prognosis, so screening programs were developed which includes clinical and self breast examination, mamography, genetic screening, ultrasound and MRI.
In every disease should search for the best way for treatment and in breast cancer it divided into locoregional treatment, and systemic treatment (Chemotherapy and hormonal therapy) (surgery, radiation).
Despite impressive progress in diagnosis and treatment a substantial fraction of women fail conventional treatments, HER2/neu targeting monoclonal antibodies such as trastuzumab given as adjunct therapy have significantly improved the relapse free survival of breast cancer patients.
A special type of breast cancer which we intend in our study to talk about it which is inflammatory breast cancer which called like this because the affected breast displays the same symptoms that may occur with inflammation like swelling, redness, histopathological studies on IBC cases revealed the lesion to be an undifferentiated carcinoma with widespread carcinomatosis of the dermal lymphatic vessels. In Egypt, the incidence of IBC has been reported to be from 6-9% of all breast cancer cases with an incidence that is on the rise.
IBC also diagnosed by triple assessment but it is characterized by the presence of cancer cells in the subdermal lymphatics on skin biopsy, as regard treatment, multimodal therapy including chemotherapy with combination of several agents, radiation treatment, hormonal therapy and in some case surgery.
Immune cells including macrophages and T lymphocytes are known to infiltrate various type of tumors including breast thus contributing to high levels of growth factors, hormone and cytokines in the tumor microenviron-ment, different methodological approaches have been used including flow cytometry, polymerase chain reaction, in our study we concentrate to do immunophenotyping by flow cytometry of IBC on non-IBC patients to identify IBC tumor infiltrated immune cells and relationship to the prognosis.
Finally after completing our study on fourty female patients with IBC and non IBC, it has been suggested according to our results that IBC patient have significant increase in the mean percent of natural killer (NK) cells and monocytes/macrophages isolated from the blood collected form the tumor microenvironment.
Finally, infiltration of immune cells within the tumor microenvironment may be associated with poor prognosis.
