Abstract

The present work aims to study and evaluate angiogenin as predictor and prognostic of Intraventricular hemorrhage in preterm infants.
This case control study was conducted on 80 neonates, 40 preterm neonates & 40 healthy full term neonates chosen from maternity hospital at Ain Shams University Hospitals.
Preterm neonates with intraventricular hemorrhage (IVH) were 6 neonates, their gestational age ranged from 28-32 weeks, gestational weight ranged from 800-2800 g, 4 males & 2 females (80% males, 20% females) 50% of which delivered vaginally & 50 % by caesarean section.
Neonates without IVH were 74, 40 healthy full term neonates their age ranged from 37-41 weeks and gestational weight ranged from 2200-4100g and 34 preterm neonates their age ranged from 28-34 weeks and their gestational weight ranged from 1000-2800 g.
Athorough history was taken (prenatal, natal, postnatal).The neonates were subjected to Apgar score at 1and 5 minutes,assessment of gestational age through analysis of maternal dates,thorough complete clinical examination including cardiac, chest, abdominal and neurological, laboratory evaluation of angiogenin in cordblood was performed by ELISA techniqueand neonatal cranial ultrasound to detect IVH.
Serum angiogenin level was statistically significant higher in preterm neonates (median ± IQR 200.00 ± 235ng/ml) compared to full-term neonates (median ± IQR 22.00 ± 10ng/ml).
No Statistical significant difference regarding serum angiogenin was found between preterm neonates with IVH (median± IQR 250.00 ± 300ng/ml) and preterm neonates without IVH (median ± IQR 200.00± 225ng/ml).
Serum angiogenin level shows no correlation with gestational age, birth weight, sex & mode of delivery.
The equation of logistic regression shows that there is no significant predictive value of the mean serum Angiogenin values for occurrence of IVH in PT neonates butApgar at 5 minutes and gestational age show significant predictive value for the incidence of IVH in preterm neonates.
Roc curve revealed that angiogenin is neither sensitive nor specific for the prediction of IVH in neonates.
