Abstract

There are many factors that are responsible for hair damage. These factors can be classified into two categories, namely internal factors and external factors. Internal factors that influences hair condition include hormonal, genetic factors and external factors from outside our body for example medications, disease, nutritional and environmental factors as exposure to ultraviolet rays.
This study aimed to detect the ultra-structural changes of hair shafts and follicles after ultraviolet irradiations used in the treatment of some dermatological diseases.
It is a case control study on twenty female participants with age range from 20-40 years who were compared to ten age matched ,female normal volunteers. The thirty participants were divided into three groups, first group Included ten vitiligo patients receiving PUVA therapy, second group Included ten vitiligo patients receiving narrow band-UVB therapy and the third group included ten normal volunteers served as control. A biopsy was taken from each participant the scalp with its hair shaft and follicle to study the changes under Transmission electron microscope.
Clinically, we observed splitting at the ends of the hair shaft, dryness, reduced strength, rough texture, loss of luster and the shaft was lighter in colour.
Histopathological changes in the hair follicles seen under the light microscope in the form of reduced hair follicle thickness with shrunken hair bulbs, perifollicular inflammatory cellular infiltrate were seen, hyalinised and disorganized collagen, cortical cells showed swelling and vacoulization of cytoplasm, condensed nuclei, it also showed distorted thick follicle with loose laminated keratin and compressed dermal papillae.
Statistical analysis of the electron microscope results revealed a highly significant difference between the 3 studied groups regarding the decrease in melanocytes number , swelling, vacuolization and protrusion of cytoplasm , necrosis and reduced thickness of hair follicles , lysis of the plasma membrane. The changes were also highly significant regarding the presence of shrunken bulbs, cellular infiltrate ,hyalinised and disorganized collagen of hair follicle. There was no significant difference between the 3 groups regarding partial and complete apoptosis.
