Abstract The purpose of this study is to assess the prevalence and predictors of hepatic insulin resistance (IR) in treatment naIve, nonncirrhotic, non-diabetic patients with CHC V s. CHB and to assess the effect of achieving sustained virological response (SVR) on IR (hepatic and peripheral), insulin secretion, lipid profile and FF As in patients with CHC found to have pre-treatment hepatic IR (HOMA > 2.1 ). This study was conducted on 171 patients with biopsy-proven chronic hepatitis C (120) and chronic hepatitis B (51) and with no history of chronic alcoholism, other liver diseases or active immunological diseases. Patients were selected from the out-patient liver clinic of Toronto Western Hospital, Canada. * ALL enrolled Patients were subjected to:- - Complete history taking and thorough clinical examination - Body fat assessment through measurement of their BMI and waist circumference - Laboratory investigations including:- (1) Routine investigations. a- Liver enzymes AL T &AST. b- Serum fasting blood glucose and insulin. C- Lipid profile and serum FF As. d- Quantity of serum HCV RNA & HBV DNA. e-Liver biopsy for assessment of inflammatory grade (Metavir), fibrosis (Laennec’s) and steatosis. |
