Abstract

The objective of this work was to define the frequency and severity of steroid related behavioral side effects in children with steroid sensitive idiopathic nephrotic syndrome (SSNS) during Treatment for relapse. For this purpose, 40 pediatric patients with steroid sensitive nephrotic syndrome were studied group I include 30 children known as SSNS at complete remission or low dose of prednisolone and have relapse on follow up; 20(66.7%) are male and 10(33.3%) are female their age ranged from 6.0-14.0 years and group II include 10children known SSNS at complete remission but not entre in relapse at time of follow up ; 7(70%) are male and 3(30%) are female their ranged from 8.0-15.0years.All children in this study were subjected to full history taking, thorough clinical examination, assessment socioeconomic standard, and assessment of pediatric quality of life, a battery of psychometric tests included pediatric anxiety, depression, and aggression scores. Results of our study show that there is highly significant increase in the mean values of anxiety, depression and aggression behavior among cases starts to appear on 1st week and extends to third, fifth and seventh weeks compared to baseline and control. With mean value of prednisone dose among cases at base line is 0.25mg/kg/AD, while at first week and third week are 2 mg/kg/D, fifth week is 1.7mg/kg/AD and lastly during seventh week is 1.4 mg/kg/AD. And mean value of albumin levels among cases at baseline is 4.2 gm/dl, while at first week is 1.5gm/dl, third week is 2.6gm/dl, and fifth week is 3.5gm/dl and lastly during seventh week is 4.4gm/dl. Our study show the seven weeks of follow up among cases show significant positive correlation between prednisone doses and mean values of anxiety and depression scores and aggression (P<0.05). The results clarify that, there are no statistical significance differences as regard baseline scores of anxiety, depression and aggression among low(2mg%) and normal albumin values. The seven weeks of follow up among cases show no significant correlation between albumin levels and mean values of anxiety, depression and aggression scores (P>0.05). Our study shows that there are highly significant reduction in the mean values of albumin among cases starts to appear on week one and extends to three and five weeks compared to baseline, while on seven weeks the mean values of albumin are significantly higher compared to baseline among cases. Results of the present work revealed that all studied children with SSNS often experience significant problems with anxiety, depression, and increased aggression during high dose steroid therapy.
