Abstract

Summary
Neonatal jaundice is a cause for parents and physicians’ anxiety due to its neurological complications and prolonged hospital stay with increase cost. With early hospital discharge practice, neonatal hyperbilirubinemia has become an important cause for hospital readmission.
Haptoglobin is a plasma α2- glycoprotein which binds free hemoglobin thus preventing oxidative damage; haptoglobin is an integral part of acute phase reactant in term neonates.
This study was aiming to detect if cord blood haptoglobin can be used as an early predictor of jaundice in normal full term newborns.
It was designed as a case – control study including forty neonates This study was conducted in a prospective random way in the delivery room of Gynecology and Obstetrics department and neonatal intensive care unit(NICU)of mansoura general hospital and a blood sample was taken from the umbilical cord immediately at delivery room and analysed for total and direct serum bilirubin ,CBC,blood groupand haptoglobin.A blood sample is also taken from the mother for detection of blood group of the mother and RH. Another blood sample was taken from the baby at the fifth day of birth and analysed for total &direct serum bilirubin&haptoglobin.
Data was collected , tabulated and statistically analyzed.
The results of this study revealed that :
Mothers aged between 18 and 36 years with the average of 25.45±4.65.their gestational age was 39.5±1.5.Number of deliveries was between one and five with an average of 1.33±1.21.Twenty eight mothers have normal delivery (70%)and twelve mothers having cesarean section delivery(30%).Birth weight of the babies were measured as 3.270±274.05g in average.There was no case delivered by vacuum extraction .All cases were breast fed after delivery.The whole blood and biochemical values of the mothers and babies durig control were within normal limits . bilirubin value in the fifth day was between 1.8 and 18.5mg/dl.UC blood serum haptoglobin value was between 1.2-13 (average 4.77 ± 4.22 ).Fifth day serum haptoglobin value was between 23.00 - 144.70 (average 66.74 ± 43.84 ).
In the current analysis correlation test between UC blood serum haptoglobin value and fifth day serum bilirubin value demonstrated statistical significance (r=-0.386 ,p= 0.014 ).
