Abstract

Natural Orifice Transluminal Endoscopic Surgery (NOTES) is an emerging
technology that uses endoscopic instruments passed into the peritoneal cavity through
hollow viscera (transgastric, transcolonic, or transvaginal) to perform surgical
procedures without the use of abdominal incisions. Various potential benefits of this
novel technique have been proposed. The benefits of less invasive access could
completely avoid incision-related complications such as wound infection, adhesions,
hernia and postoperative pain ,with shorter recovery time and lower complication
rates. Published clinical trials on human include successful NOTES cholecystectomy
,appendectomy, peritoneoscopoy, liver biopsy and nephrectomy although most of
them are hybrid NOTES . They represent a great success for the new technique. The
cycle of development of surgery is continuing to provide a desirable new technique
that conceives the available minimal access approach and provides significant
improvement in its advantages. Advances in technology and training are important
tomake NOTES safe and practical. Taking into consideration the active research and
proposed benefits of NOTES over other techniques, evolution of current surgical
practice to NOTES is anticipated to provide best surgical care. Irresistible desire to
provide scarless surgery is driving modern surgery to a new era in spite of all current
criticism to NOTES.
