Abstract

This study aimed to study the level of thrombopoietin in serum of cirrhotic patients in different age groups in an attempt to find out the effect of age on thrombopoietin level in cirrhotic patients.
This study was conducted on 45 patients with liver cirrhosis (child A, B) diagnosed by clinical examination, liver biopsy, they were classified into three groups:
Group I: 15 patients below the age of 20 years.
Group II: 15 patients between the age of 20 years to 40 years.
Group III: 15 patients above the age of 40 years.
In addition to 15 healthy volunteers matching the age groups.
Exclusion criteria:
Hepatocellualr carcinoma, splenectomy, hematological disorders such as aplastic anaemia, leukemia and idiopathic thrombocytopenic purpura, disseminated intravascular coagulopathy, drugs that may affect platelets, metabolic liver diseases, chronic renal failure and lone schistosomal affection.
Our results showed that:
- Regarding age, the mean of age was 18.731.223 years for group I. 36.23.57 years for group II, 58.738.455 years for group III, the mean of age for control group was 32.1312.671 years.
- Group I include 5 (33.3%) males and 10 (66.7%) females, group II include 3 (20.0%) males and 12 (80.0%) females, group III include 4 (26.7%) males and 11 (73.3%) females, control group include 5 (35.7%) males and 9 (64.3%) females. There were no statistical significant differences between the different studied groups regarding sex.
- There were no statistical significant differences between the three studied groups regarding pallor, jaundice, L.L oedema, Flappy tremors, palmar erythroma, spider Nevi, purpra, gynecomastia.
- There were no statistical significant differences between the three studied groups regarding liver size, splenic size, ascites and causes of liver cirrhosis.
- There were statistical significant differences between the three studied groups regarding child pugh score.
- The mean of Hb were 11.1270.89, 10.7730.9838, 9.860.9148 and 13.2333.012 gm/dl for group I, II, III and control respectively. Control group have values statistically higher than other groups regarding haemoglobin.
- WBCs count was 6.5272.262, 6.362.49, 6.1722.49 and 8.3871.33 (X103/cmm) for group I, II, III and control respectively. Control group have values statistically higher than other groups.
- Platelet count was 108.9522.01, 96.8326.661, 69.6328.825 and 705.67996.167 (X103/cmm) for group I, II, III and control respectively. Control group have values statistically higher than other groups.
- There were statistical significant differences between the different studied groups regarding bleeding time, clotting time and prothrombin activity.
- There were no statistical significant differences between the different studied groups regarding bone marrow.
- There were no statistical significant differences between the different studied groups regarding cause of cirrhosis.
- The mean of AST was 52.1930.505, 49.0121.994, 51.2425.999 and 24.4814.44 IU/L for group I, II, III and control respectively. Control group have values statistically lower than other groups.
- The mean of ALT was 63.42732.7388, 55.27324.3817, 53.6426.7718 and 21.9734.3376 IU/L for group I, II, III and control respectively. Control group have values statistically lower than other groups.
- The mean of GGT was 63.9350.679, 36.7315.563, 48.7716.232 and 23.628.333 IU/L for group I, II, III and control respectively. Control group have values statistically lower than other groups.
- The mean of Albumin was 3.40.1558, 3.2670.282, 2.520.8046 and 4.0270.339 gm/dl for group I, II, III and control respectively. Group III have values statistically lower than other groups.
- There were no statistical significant differences between the studied groups regarding bilirubin.
- There were no statistical significant differences between the studied groups regarding PV.
- o. varices were found in 2 (13.33%), 5 (33.33%) and 10 (66.67%). varices grade I were found in 2 (13.335), 2(13.33%) and 3 (20.0%). Portal hypertensive gastropathy were found in 1 (6.675%), 3 (20.0%) and 4 (26.67%) for group I, II and III respectively. There were no statistical significant differences between the different studied groups regarding o.varices.
- The mean of Thrombopoietin level was 117.3353.021, 102.7340.335, 80.3336.536 and 224.7721.99 pcgm/ml for group I, II, III and control respectively. Control group have values statistically higher than other groups.
- There were negative significant correlation between age, TPO (P=0.010) and age with P.V. (P=0.010).
- There were positive significant correlation between platelet, TPO (P=0.03) and negative significant correlation between platelets with P.V. (P=0.000).
- There were negative significant correlation between bleeding time with TPO (P=0.000) and positive correlations between bleeding time with AST, ALT and P.V. (P=0.001, 0.001 and 0.009 respectively).
- There were negative significant correlation between clotting time with TPO (P=0.000) and positive correlations between bleeding time with AST, ALT and P.V. (P=0.030, 0.048 and 0.006 respectively).
- There were positive significant correlation between PT with TPO (P=0.000) and negative correlations between PT with AST, ALT and P.V. (P=0.001, 0.000 and 0.000 respectively).
