Abstract

Methicillin-resistant Staphylococcus aureus (MRSA) has emerged worldwide as a nosocomial pathogen of major importance, and the incidence of infections caused by MRSA continues to increase.
The existence of drug resistant strains of MRSA has made it increasingly difficult to treat serious infections caused by this organism.
The conventional agent for therapy of MRSA infections is vancomycin. However, there are several limitations of treatment of MRSA infections with vancomycin, and its role in the management of serious infections is being reconsidered.
The presence of vancomycin resistant strains allows usage of other antibiotic to treat MRSA infection as teicoplanin, linezolid and even tigecycline but some resistant strains to those antibiotic appear. This allows trial of usage of daptomycin.
The purpose of this study was to compare the effect of daptomycin, vancomycin, linezolid and tigecycline on MRSA isolated from patients admitted to surgical intensive care units in Ain Shams University Hospital.
The present study was conducted on 115 patients admitted to different intensive care units of surgery department of Ain Shams University Hospitals during the period from March 2011 to October 2011. They were 68 male and 47 female .The age of the patients ranged from 29 to 66 years from whom 29 MRSA were isolated from different clinical specimens as blood, endo-tracheal aspirate, sputum, burn exudate, and pus from wounds. Identification of the isolate was then done by conventional bacteriological methods. The minimal inhibitory concentration (MIC) of daptomycin, vancomycin ,linezolid and tigecycline was detected using the E-test .
The present study revealed the good in vitro activity of daptomycin against MRSA infection in comparison with linezolid and vancomycin. All isolates showed 100% susceptibility to daptomycin , linezolid and vancomycin while only 38% were susceptible to tigecycline.
