Abstract This study was carried out to study different mechanical ventilator modes (SIMV, SIMV + PS versus APRV) outcome in AECOPD with respiratory failure, trying to identify possible role of APRV in these patients. For this purpose, 45 patients (29 males and 16 females with a mean age of 61.8±7.6 years) with acute exacerbation of COPD admitted at the Respiratory ICU of Chest Department, Zagazig University Hospitals were included. They were classified according to the mode they received into 3 groups. Group I: They were 15 patients (33,3%) 10 patients (66.67%) of them were males and 5 patients (33.33%) were female ( 12 patients were ventilated from the start by SIMV+PS,while 3 patients changed from APRV mode during 1h because thay can,t tolrated the mode one of them devolped tachycardia(HR>110) and other two patients devolped tachypnea (RR>45) This patients were put on SIMV mode plus PSV (15-22cm H2O) according to the protocol used in our respiratory ICU at Zagazig University Hospitals . Group II: They were 15 patients (33,3%) 10 patients (66,7%) of them were males and 5 patients (33,3%) were female This patients were put on SIMV mode according to the protocol used in RICU at Zagazig University Hospitals Group III: They were 18 patients(3 patients(2 males ,and 1females) failed to complete on this mode and changed to SIMV+PS during 1h) while the other 15 patients was complete on APRV mode 9 patients. |
