Abstract

Alumina on Alumina ceramic bearings were first used in THA in France in 1970. The quality of alumina ceramic and the design of prostheses have improved over time. An alumina-on-alumina combination is now safe and several teams from around the world have shown this. As expectations for durability of total hip arthroplasty have increased and indications for the operation have been extended, it has become obvious that the bearing surface will be the ultimate determinant of implant survival. The potential advantages of using a ceramic on ceramic articulation for total hip arthroplasty can be quickly summarized as decreased wear and elimination of osteolysis. Osteolysis from wear debris is commonly viewed as the major obstacle blocking the development of a “lifetime” hip replacement. The need to eliminate wear and osteolysis has been magnified by the extension of indications for total hip arthroplasty to younger, more active, healthier patients with long life expectancies. The advantages largely exceed the potential disadvantages. Fracture of the components, which was once the main fear for both patients and surgeons has declined to an acceptable level, although it can still occasionally occur. The younger, more active patients are the target population as they require an extended survivorship for their arthroplasties.
Objectives: This study aims to review Ceramic on Ceramic total hip prosthesis as regard biomechanics, surgical technique, advantages and complications of ceramic on ceramic total hip arthroplasty.
