Abstract

Scalp burns can result in significant burn alopecia. The management of these problems depends on the location and extent of the alopecia. The reconstruction of post burn scarred areas was achieved using simple techniques like skin grafts and local transposition flaps. Later on, more sophisticated methods, like free tissue transfer, were employed. The disadvantages of these methods (graft retraction, donor side morbidity, graft loss, suboptimal aesthetic results) favored the development of another reconstructive method, the tissue expansion. Skin expansion constitutes an excellent option in the treatment of burn sequelae especially in the scalp. This work aimed to study the use of tissue expansion for reconstruction of post burn alopecia and included 30 patients presented by post burn alopecia and treated by tissue expansion, the study included 2 parts, part one: retrospective study since January 2010 till October 2011 and part two prospective study from October 2011 till August 2012. Tissue expansion is a two stage procedure the first was to apply the expander in a healthy tissue under the galea to increase the mitotic activity of the skin to produce a flap for coverage of the scarred area. The operation was done under general anesthesia and complete aseptic technique, paralesional incision was done with a pocket for the expander application and external reservoirs were used with proper drainage and wound closure in two layers (the galea and the skin Inflation started soon when wound was stable till the desired flap length was obtained and proceed to the second stage where the expander was removed and replacement of the post burn alopecia segment was done by advancement or designing a flap. Follow up of the patients was done, data were collected and results were obtained. Reports of complications in the use of tissue expansion are numerous in this study Complications were defined and classified as major complications (infection and expander extrusion) In which expansion process was stopped with removal of the expander and minor complications (Seroma, haematoma and wound dehiscence) Which did not interrupt the expansion process and can be solved . from this study, it could be stated that tissue expansion provides flaps which provides skin of color and texture similar to the surrounding skin, the presence of adnexes and sensitivity without damaging the donor site. It also allows the harvesting of large full skin with primary closure of the donor site, Flaps are stable at a late follow-up, and the procedure may be repeated when necessary (reexpansion).
