Abstract

This work was planned to assess the results of treatment of displaced supracondylar fractures of the humerus in children in prone positioning. The patients were treated by closed reduction under image intensifier and percutaneous criss-cross pinning by two kirschner-wires one through the medial epicondyle and one, sometimes two through the lateral epicondyle while the patient in prone positioning. When the elbow was very swollen, a small medial incision was made over the medial epicondyle.
The material of this study included 20 cases of displaced supracondylar fractures of the humerus.
The age of the patients ranged between 3 years and 9 years with a mean (5.61.6) years.
There were 15 boys (75%) and 5 girls (25%) with a ratio of about 3:1.
All cases of the material were examined clinically and radiologically there were all of extension type.
The left elbow was affected in 15 patients (75%) and the right elbow was affected in 5 patients (25%).
The time lapse before surgery ranged between 1.5 to 4 days with average 2.7 days. there were 12 patients operated within 3 days of injury (60%), and 8 patients operated after 3 days of injury(40%).
In all but two cases the posterior slab and the wires were removed after four weeks of surgery, in the remaining two cases wires and slab were removed after six weeks.
The follow up period ranged from 3 to 14 months with a mean of 5.4±2.2. Patients were followed up clinically and radiographically every 4 weeks during the period of follow up.
At the end of the follow up period there was;
 11 cases excellent (55%)
 7 cases good (35%)
 2 cases unsatisfactory (10%)
The factors that had an effect (significant correlation) on the final clinical results in this study was the quality of reduction as the more perfect the reduction the better were the results and duration of follow up as the longer the follow up the better the results.
The complications encountered in this study were;
 Ulnar nerve injury in one case and the patient was lost for follow up and not included in the statistical analysis.
 Pin tract infection in one case, which resolved after removal of the pins within 10 days.
 Lack of extension more than 20 degrees in two cases
