Abstract End-stage renal disease (ESRD) is a major health problem in the world, including Egypt. Diabetes mellitus and hypertensionare leading causes of ESRD in most countries. A public health model that integrates health promotion and disease prevention is necessary to handle this major health problem. Methods: uncontrolled clinical trial carried on 62 patients selected from Zenhom PHC. Results: Knowledge level of studied subjects was poor but after implementation of the educational message, a significant improvement was revealed in patients’ knowledge and attitude with lowering of their mean levels of blood pressure and HbAlc.
