Abstract

Abstract
Background and Purpose: Carotid atherosclerosis ranges from diffuse intima media thickening to total occlusion, and the severity depends mainly on presence of traditional cardiovascular risk factors, some patients may develop stroke and transient ischemic attacks. This article describes the prevalence of extracranial carotid atherosclerosis assessed by ultrasonography, its association with risk factors, and its relation to symptomatic TIAs and stroke in Egyptian population.
Materials and Methods: In a retrospective manner, carotid duplex findings were correlated.with risk factors and clinical presentation in 4629 subjects [mean age 55.96±12.31, 3356(72.5%) were men] referred to three vascular laboratories between 2003 and 2008. The study population is classified according to duplex finding into athersclerotic /normal groups and according to cerebrovascular presentation in to symptomatic/ asymptomatic groups. Each group had data about age, sex and traditional risk factors for atherosclerosis. Multiregression analysis was done to detecte the independent predictors for atherosclerosis, stenosis severity and symptomatic presentation.
Results: Carotid atherosclerosis was present in 43% of the study population (50 - 70% carotid stenosis in 3.3% and ≥70% stenosis in 1.4% of people). 62% of patients with carotid atherosclerosis were asymptomatic. Age>50 years ,associated ischemic heart disease, diabetes mellitus, hypertension , dyslipideamia , smoking , male gender, and prescence of 3 or more risk factors were independent predictors for carotid atherosclerosis . Age>50 years , and prescence of 3 or more risk factors were independent predictors for both haemodynamic carotid artery stenosis ≥ 50% and severe carotid artery stenosis ≥ 70%. Obesity, ≥ 70% carotid stenosis , hypertension , age>50years , ≥50% carotid stenosis, dibte mellitus ,and smoking, are independent predictors for cerebro-vascular symptoms.
