Abstract

The field of noninvasive imaging of myocardial viability remains active and exciting, as old techniques are refined and applied to the improvement of patient care and new techniques emerge that present clinician investigators with new challenges to evaluate their worth and cost-effectiveness in this era of managed care and cost reductions.
Low dose dobutamine echocardiography is used in the detection of viable myocardium. In addition, gated SPECT could evaluate regional wall motion, even in areas inadequately assessed by echocardiography and in the identification of viable myocardium.
201-Tl Scintigraphy SPECT is used as a gold standard in assessment of myocardial viability.
Tetrofosmin SPECT scan is used to assess the myocardial viability in comparison with the 201-Tl Scintigraphy SPECT with sensitivity 98%, specificity 100%, PPV 100% and NPV 93.5%.
Low dose dobutamine echocardiography is used to assess the myocardial viability in comparison with the 201-Tl Scintigraphy SPECT with sensitivity 60.6%, specificity 100%, PPV 100% and NPV 30.8%.
Tetrofosmin SPECT scan gives high concordance rate = 96.2% with thallium scan in the perfusion segments assessment, however give concordance rate = 86.2% in wall motion assessment.
Low dose dobutamine echocardiography gives concordance rate = 64.8% in wall motion assessment in comparison with the gold standard thallium SPECT.
201-Tl gated SPECT rest-redistribution is used as a gold standard in assessment of myocardial viability.
Low dose dobutamine echocardiography is used in the detection of viable myocardium by assessing the contractile reserve.
Gated SPECT could evaluate regional wall motion, even in areas inadequately assessed by echocardiography for identification of viable myocardium.
Nitrate enhanced Tc 99mTetrofosmin SPECT demonstrate excellent results comparable to that of TI201 .
Tetrofosmin SPECT may be superior to low dose dobutamine echocardiography in evaluation of myocardial viability; however both are considered complementary.
