Abstract

Neonatal jaundice is a common occurrence in newborns. Most cases of neonatal jaundice have a benign course; Jaundice is observed in 1st week of life in approximately 60% of term infant and 80% of preterm infant.
The accurate measurement of bilirubin concentrations is essential for the diagnosis of hyperbilirubinemia and for guiding the clinician with regard to treatment.
Transcutaneous bilirubinometry measures the intensity of yellow color in the skin and subcutaneous tissue and correlates it with the serum bilirubin concentration in newborn infants. It is reported to be safe, simple, objective, reproducible, cost effective, noninvasive modality in the screening and monitoring of jaundiced newborn infants.
Hence, the aim of this study was to evaluate the accuracy of TcB using the Minolta Air-Shields JM-103 device in a specific population of preterm newborns with a gestational age of (30–36)weeks, and to identify the most appropriate measurement site and compare it with total serum bilirubin.
Our study was conducted on 100 preterm infants were admitted in the neonatal intensive care unit (NICU) of Shebeen ELkom Teaching Hospital.
THE CASES WERE CLASSIFIED ACCORDING TO:
●According to the gestational age we classified the neonates to:
Group І: early preterm which include 49 preterm infants of gestational age ranged from (30-33) weeks, they were 30 males and 19
females with a mean weight 1.6±0.4 Kg.
Group П: Late preterm which include 51 preterm infants of gestational age ranged from (34-36) weeks, they were 29 males and 22 females with a mean weight 2.4±0.5 Kg.
●According to age at time of test:
Group I a preterm infants > 72 hours of age, which include 37 preterm infants, they were 19 males and 18 females.
Group IIa preterm infants < 72 hours of age, which include 63 preterm infants, they were 40 males and 23 females.
TcB measurement of jaundice using the Minolta Air shields Jaundice Meter JM-103 (Osaka, Japan) were done immediately before or within 10 min after blood sampling for plasma bilirubin concentration measurements the probe was placed against the forehead, sternum and knee of the infant in a supine position, the three reading and the mean of the three reading of each infant were recorded and compared with total serum bilirubin, measured by Colorimetric method.
The results of this study revealed that there is statistically significant correlation between total serum bilirubin with forehead,
sternum, knee and the mean of three reading of transcutaneous jaundice meter in early and late preterm infants what ever gestational age, birth weight and post natal age.
It also revealed that the mean of the three reading is better than each site alone in all preterm infants.
