Abstract

Introduction: cancer with pregnancy is the 2nd leading cause of death during the reproductive years complicating between 0.02% to 0.1% of pregnancies . this incidence is expected to rise with the increase in the age of childbearing. the most common types of cancer with pregnancy including cervical cancer, breast cancer, malignant melanoma, leukemia and lymphoma.
Aim of the work: to treat the curable malignant disease of pregnant women and to protect the fetus from harmful effect of cancer treatment.
Diagnosis of cancer with pregnancy: A common cause of delay in the diagnosis of cancer during pregnancy is the mistaken attribution of symptoms to the pregnancy itself. The physiological changes in pregnancy include increased breast density and size, altered biomechanical stress on the spine, and abdomino-pelvic compression, all of which increase as the pregnancy progresses and can cause nonspecific maternal symptoms. Other common features of pregnancy include nausea and vomiting, particularly in the first trimester .Cancer diagnosis requires tissue sampling and cytological examination. Most routine modes for tissue sampling, including open biopsies under local anesthesia, can be safely performed during pregnancy without harming the mother or the fetus.
Treatment of cancer with pregnancy: depend on the sit of tumor, stage of tumor, type of pathology, gestational stage and the desire to continue this pregnancy or not.
Treatment including surgery, chemotherapy radiotherapy and supportive treatment.
Chemotherapy in pregnancy: the physiological changes that occur during pregnancy including increase plasma volume, renal clearance, the third space created by amniotic fluid and faster hepatic oxidation lead to decrease active drug concentration in pregnant than nonpregnant.
the effect of chemotherapy on fetus depend on fetal stage and total dose.
Radiation in pregnancy: the effect of radiation during pregnancy depend on radiation dose, fetal stage, the machine and its leakage, the size of filed and the distance from the fetus.
Its effect including: fetal death, malformation, genetic effects and carcinogenesis.
