Abstract

Our study was done in Assiut, Minia and Ain Shams University hospitals, Department of Orthopaedics from September 2007 till September 2010. In this study we had 13 patients with 15 hips needing a primary THA surgery. No patient was lost in the follow. The mean age at the replacement surgery was 31.6 years old. There were 53.3% females and 46.7% males. Follow-up ranged from 12-44 months.
In our study we had no categorized groups. All patient were carefully examined detailed history was taken from them. Radiological studies routine X-rays in antero-posterior and lateral position of the hip needing surgery.
The cause of surgery in our study was 9 hips with AVN, 4 with OA (3 primary and 1 secondary), 1 hip mono-articular RA and finally 1 with AS.
We use the cementless Proxima stem in our cases. We used cementless cups, the Duraloc cup in 12 cases and Pinnacle cup in 3 cases. Head size 28mm in 12 cases (all with Duraloc cup) were used and 36mm head size in 3 cases (with Pinnacle cup).
We had 8 hips of MOPE bearing surface, 4 cases of Biolox forte COC and 3 cases of Biolox delta ceramic head over UHMWPE. The later 3 cases were done with Pinnacle cup and 36 mm head.
We did acetabular reconstruction in 1 case using the harvested head and two fully threaded screws. The graft was incorporated.
No case with other types of THA except one case did conventional stem 4 months later to Proxima stem insertion.
All the patients were followed-up and assisted using the Harris Hip Score. Strict instructions were given to all candidates. Post operative exercises were instructed carefully to all patients.
Our results were satisfactory and comparable to others. Our final results according the (HHS) were 85.9 mean score compared to 19.9 preoperative mean score.
We had 1 case of early anterosuperior dislocation but she was treated at once and she did not dislocate again. Another case of early deep infection underwent serial debridement with local antibiotic used; now she is satisfied by this operated hip and seeks for operating the contralateral affected hip by AVN. Also 1 case with LLD and 3 cases with femoral periprosthetic fracture.
