Abstract

Congestive heart failure is a serious disease and if left untreated may cause death. However, with proper nursing care may be controlled and provide comfort to the patient. Patient teaching is probably one of the most important factors in nursing management of congestive heart failure. Recurrent episodes of cardiac failure are often due in part to non-compliance, such as failing to follow medication therapy, failing to follow dietary guidelines, missing medical appointments, engagement in excessive and unregulated exercise, and failing to recognize recurring symptoms. The aim of this study was to determine the impact of a protocol of care about congestive heart failure on patients, physical responses and compliance to therapeutic regimen. Physical responses including vital signs, dyspnea, edema, fatigue, skin conditions, laboratory finding and drug side effects. The study was carried out in medical department of Shebin El-Kom Teaching and University Hospital in Menoufiya Governorate. The data collection phase was conducted over 11 months. The study sample constituted sixty patients of both sexes. They were selected according to the following criteria 1) Having a confirmed diagnosis of congestive heart failure b) Having a hospital stay of at least 30 days. C) Conscious and willing to participate in the study. These patients were divided into two groups each group composed of 30 patients. Subjects were randomly assigned to one of the two groups the studied group received the designed protocol of care about heart failure while the control group was left for the routine care provided by the hospital staff. The following instrument were used to collect data: Demographic / clinical data sheet. Patient pre-knowledge questionnaire (PPKq). Observational checklist. Physical responses monitoring sheet (PRMs). Questionnaire sheet to assess patient’s compliance to the prescribed regimen. The main findings of this study showed that: Regarding sex, almost half of the control and the studied group were males (56.7% 50% respectively). Age, 26.7% of the control group and 23.3% of studied group fall within 50 and 60 years and above. The majority of subjects in the control and studied groups (80%, 73.3% respectively were married, 70% of the control group and 80% of the studied group were illiterate. The majority of the control and the studied group, have manual work (33.3% and 36.7% respectively). The findings related to the total knowledge score of patients about the nature of heart failure showed a significant difference between control and studied groups after the care. In relation to the total knowledge score about the rest and exercise. There was a statistically significant difference between control and studied groups after the care.
