Abstract

Reproductive failure is a major medical and social problem and has received a considerable attention operative hysteroscopy has abenifiecial role in the management of reproductive failure due to either submucous fibroid,septate uterus PTO or interauterine psynechiae.
The aim of this study is to evaluate the outcome of operative hysteroscopy in these patients with reproductive failure.
This retrospective study was conducted in the department of Obstetric and Gynecology, Suzan Moubark University Hospital during the period from2005-2009.
Ninghty two women were included in the study, as having the following inclusion c iteria:
-Age:25-35
-The clinical picture and eitiology of reproductive failure were due to conditions that could be treated by operative hysteroscopy and included submucous fibroid,septate uterus,interautrine psynechiae and proximal tubal obstruction
Detailed medical and operative history followed by general and pelvic examination was done for those patients.
Combined laparoscopy and hysteroscopy was done.
Hysteroscopic myomectomy was done for thirty five cases.
Hysteroscopic septal resection was done for 17 cases
Hysteroscopic tubal cannulation was done for 15 cases
Hysteroscopic adhesolysis was done for 25 cases
The results of the study were encouraging.the role of the operative hysteroscopy in management of reproductive failure which is a none invasive technique with less morbidity and reduced period of hospital stay aMong 35 cases underwent hysteroscopic myomectomy 22 cases got pregnant 10 cases of them achieved term pregnancy ,5cases had PTL and 7 cases had experieced 1st and 2nd ttrimester abortion.
aMong 17 cases underwent hysteroscopic metroplasty 10 cases got pregnant 7 cases of them achieved term pregnancy 2cases had PTL and one case had experieced 2nd ttrimester abortion.
aMong 15cases underwent hysteroscopic tubal cannulation 9 cases got pregnant8 cases of them achieved term pregnancy no cases of PTL and no cases of abortion but ectopic pregnancy was recorded in one case.
aMong 25cases underwent hysteroscopic adhesolysis 10 cases got pregnant 6cases of them achieved term pregnancy 2cases had PTL and 2 cases hadabortion
Data collected as regard the age, duration of infertility,cause of reproductive failure and the outcome of operative hysteroscopy in these cases revealed that operative hysteroscopy is a safe and less invasive procedure that preserves the endometrial and pelvic integrity with little morbidity .operative hysteroscopy should be considered the primary treatment for patients with septate uterus ,intera uterine psynechie ,submucous fibroid and PTO it has ahigh operative success rate and good reproductive outcome.
