Abstract

Clinical trials have shown that topical corticosteroids are often effective in the management of EOLP. However, tacrolimus has recently been shown to be an effective treatment of EOLP. The current study was conducted to evaluate the therapeutic effect of topical application of tacrolimus in treatment of EOLP in comparison to conventionally used topical corticosteroids.
In this double-blind randomized clinical trial, twenty patients with oral lesions consistent clinically and histologically with EOLP were participated. The patients were divided equally into two groups to receive either tacrolimus ointment 0.1% (group I) or triamcinolone acetonide in orabase 0.1% (group II). The patients were instructed to apply the treatments three times daily for six weeks followed by two weeks of observation without therapy. All patients had five visits for the clinical evaluation on weeks zero, two, four, six and eight. Each visit consisted of measuring the lesion size, and pain assessment by the VAS.
Statistical analysis of the results revealed a significant decrease in mean lesion sizes and mean pain measures in the tacrolimus group than in the triamcinolone acetonide group. In group I, seven patients were healed, two patients were relapsed, and only one patient showed no improvement. In group II, two patients were healed, three patients were relapsed, and five patients showed no improvement.
The results of the current study suggest that tacrolimus ointment 0.1% is more effective than triamcinolone acetonide in orabase 0.1% in the treatment of EOLP. However, further studies are needed to confirm the effectiveness of this treatment before it can be recommended for use as a first-line treatment for EOLP.
