Abstract

Sexuality is an important aspect of quality of life, and contrary to general opinion, women with diabetes are at increased risk for sexual problems. Female sexual dysfunction can be regarded as a silent complication of diabetes mellitus Sexual dysfunction can put a high psychological burden on patients with diabetes, and it can have a negative impact on marital relations already burdened by the presence of a chronic illness.
This study was submitted on 20 type 2 diabetic females who are in stable continuous marriage aging from 20 to 40 years old. They did not have other factors -rather than diabetes- that may affect their sexual function as gynecological, endocrinal problems, drug intake or chronic diseases or operations. Females with diabetic complications -except neuropathy- were excluded. Also pregnant, lactating females and those on contraceptive pills were as well excluded. The husbands have fare general health and sexually active.
Full history will be taken from every female including type, duration, complication & control of diabetes as well as the type of therapy she receives.
Sexual function of participating women was assessed using Arabic translation (translated by us) of Female Sexual Function Index Questionnaire designed by Rosen (Rosen et al., 2000). That evaluates Summary, Conclusions and Recommendation 123 sexual desire, arousal, lubrication, orgasm, satisfaction and pain domains.
In our Egyptian society, the attitude towards sex and talking openly about it-especially in females- is reaching the level of taboo. To the best of our knowledge only two published previous studies were done before in our community discussing the issues of female sexual dysfunction. These studies were conducted on normal and don’t include diabetic female.
So, we aimed by this work to investigate the sexual dysfunction in diabetic females living at Dakahlia Governorate.
To conclude:In diabetic Egyptian females all domains of sexual function were affected by variable degrees;
• 95% of diabetic females in this study experienced poor sexual function.
• low sexual desire was the commonest type of dysfunction among diabetic females as (100%) of participants experienced low sexual desire
• Sexual arousal disorder was experienced by 90% of participating females.
• Disorder of lubrication was found to be the complaint of 65% of
females included Summary, Conclusions and Recommendation Orgasmic disorder was experienced by 85% of participating females
• Sexual pain disorder was experienced by 65% of participating females
• Sexual dissatisfaction was experienced by 50% of participating females
• The desire and arousal are significantly negatively correlated to age (p<0.001).
• Lubrication and orgasm are significantly negatively correlated to
duration of DM (p<0.05).
• Satisfaction is significantly negatively correlated to diabetes
control (Hb A1c) (p<0.01).
• Significant difference between cases with neuropathy and nonneuropathy cases as regards arousal mean score (p<0.05).
Summary, Conclusions and Recommendation 125
Recommendation:
This study can be considered as a pilot study. It points to the role of DM in female sexual dysfunction. However, FSD is highly prevalent
in diabetic female. It is still a silent problem. The recognition of the existence of sexual problems in women with diabetes should be an invitation for clinicians to address this issue during their consultations.
Also we think that; further studies are mandatory taking into consideration to include large number of diabetic female, to design Arabic version of FSFI with cut off score suitable for Egyptians and to measure the level of personal distress associated with female sexual dysfunction.
