Abstract

The gestational trophoblastic neoplasia (GTN) refers to a spectrum of proliferative disorders of the placental trophoblast, with a wider range of histological appearances and clinical behaviors. According to World Health Organization (WHO) classification, it includes six distinct entities: Complete hydatidiform mole (CHM), Partial hydatidiform mole (PHM), Invasive mole (IM), Choriocarcinoma (CC), Placental-site trophoblastic tumor (PST), and more recently, Epithelioid trophoblastic tumor (ETT)
The incidence of GTN is higher in Asia (3.2 to 9.9 per 1000 gestations) and Latin America (0.6 to 1.1 per 1000 gestations) than in Western countries. While the incidence of GTN in North America and Europe (0.5 to 1 per 1000. On other hand, Middle East showed low ratios that generally ranged between 2 and 7 per 100,000 pregnancies. Some authorities have tried to explain the reasons of these differences on factors such as maternal age, diet, ethnicity, gravidity, poor social conditions and pelvic tuberculosis.
The diagnosis of post molar GTN may be made by clinical and/or histopathological criteria according to the current International Federation of Gynecology and Obstetrics (FIGO) recommendations which are HCG plateau for at least four values over 3 weeks.
The aim of this work is to study the recent advances in gestational trophoblastic neoplasia (GTN). We used the proliferation marker ”Ki-67” to assess its role in recent management and clinical outcomes. This is of interest not only to the histopathologist but also to the obstetricians, gynecologist and oncologist.
