Abstract

Ophthalmic surgeries in infants and children include different procedures some are elective such as; congenital and acquired cataract, glaucomas, strabismus, vitrectomy and scleral buckle and retinopathy of prematurity, others are emergency operations; the commonest of which is the open eye injuries or ruptured globe.
The anesthetist role in pediatric ophthalmic surgery is to understand the differences in anatomy and physiology between adults and pediatrics, prepare the child for the procedure which include the preoperative assessment,examination and investigations, to take into consideration the psychological aspect of this issue for both the child and his parents as well, to be aware of the anesthetic drugs and procedures and their implications on the intraocular pressure and physiology; also, to be aware of the effect of the surgical techniques and drugs used by the ophthalmologist on different systems of the child’s body with special concern about the oculocardiac reflex and its possible life threatening complications, and to provide an optimal postoperative care including analgesia and prevention of postoperative nausea and vomiting and any other distressing events that could happen postoperatively.
Although local anesthesia in infants and children is not commonly used; yet, it can be used as adjunct to general anesthesia to abort the oculocardiac reflex and to relieve the postoperative pain, nausea and vomiting but this requires caution from the anesthetist regarding the use of local anesthetic drugs due to its potential toxicity and care should also be taken regarding the different techniques as peribulbar and retrobulbar blocks and their complications
